Need/Problem Statement:

Describe the specific problem or need that your project will address.

Provide evidence (data, statistics, community feedback, etc.) that demonstrates the existence and severity of this problem/need.

Explain how this problem/need is relevant to the mission and focus areas of [Your Organization Name].

  1. Who is the target population that will be directly affected by this problem/need?

Project Plan/Methodology:

  1. Provide a detailed description of your proposed project, including its goals and objectives.
  2. Outline the specific activities and strategies you will use to achieve your project goals.
  3. Describe how you will measure the success of your project and track progress toward your objectives.
  4. Provide a realistic timeline for your project, including key milestones and deadlines.
  5. What are the potential challenges or risks associated with your project, and how will you address them?

Budget:

  1. Provide a detailed budget for your project, itemizing all expenses.
  2. Explain how each budget item is necessary and justified for the successful implementation of your project.
  3. Confirm that your total budget request does not exceed $10,000.
  4. Are you seeking additional funding from other sources for this project? If so, please provide details.

 Impact/Outcomes:

  1. Describe the anticipated outcomes and benefits of your project for the target population and/or community.
  2. How will you measure the impact of your project and demonstrate that your objectives have been achieved?
  3. How will this project contribute to positive change in the community?

 Sustainability:

  1. If applicable, explain how your project or its benefits will continue beyond the grant period.
  2. Describe any plans for future funding or support to ensure the sustainability of your work.
  3. How does this project build capacity within your organization or the community?
